He would keep both forms as long as the latter has two arms and two legs (normally) and isn't 20 feet tall. Unless you wish to remove his ability to turn into that, it's up to the player if you want the scientists to have taken that ability away. But like with other shape-shifters, he can keep his ability to change into that form. One of the main ideas behind humanisation is to turn characters that wouldn't normally get infected (i.e. transformers) into something that could get infected.

The backpacks include typical survival gear: rope, two bottles of water, two bags of trail mix, what appears to be Aspirin, a small blanket, a small flashlight with an extra pair of batteries, and a water filter either a life straw or water purification tablets. And of course an ear piece for the audio network.
If the character is lucky, they will have one of the backpacks with a hunting knife or a hammer in it but those seem to be in short supply.
Around the ship there seem to be stockpiles of trail mix, various other non-perishables, and full water jugs as well. A few other player decided items may be found as well (i.e. matches, firestarters, spam, a canteen, etc) as far as weapons go a character will be lucky to find a handheld tool, and there are no spoiling foods.
